A paixão pelo estudo das causas e dos mecanismos que atuam sobre as doenças crônicas, especialmente as Doenças Inflamatórias Intestinais, e a influência das emoções na sua evolução clínica, me motivou a escrever este livro, na tentativa de transmitir a experiência de estar junto a esses pacientes acompanhando seu sofrimento, conflitos, angústias e expectativas. Na abordagem clínica e psicológica dos pacientes pudemos perceber seu sofrimento, provocado pelos conflitos emocionais ocasionados pelas limitações e constrangimentos que os leva ao consequente isolamento, ocasionado pela sintomatologia da doença.
Muitos foram os questionamentos que surgiram durante quase duas décadas de abordagem terapêutica junto aos pacientes, adolescentes, jovens adultos e idosos, na realização de vários estudos de investigação sobre como convivem com a enfermidade em seu percurso existencial, qual a influência de suas emoções e de que forma o inter-relacionamento familiar, seja ele positivo ou negativo, interfere na evolução clínica da doença.
Um livro voltado para os profissionais da saúde, para os pacientes com DII e para aqueles que têm algum interesse em saber um pouco sobre as influências psicossociais, emocionais e ambientais na evolução clínica da enfermidade.

The passion for the pathogenesis study of chronic diseases, especially the IBD, and the influence of emotions on their clinical evolution, motivated me to write this book, in an attempt to convey the experience of being close to these patients accompanying their suffering, conflicts, anxieties and expectations. In clinical and psychological approach of these patients we were able to understand their suffering motivated by emotional conflicts caused by the limitations and constrains that leads to the consequent isolation , provoked by the symptoms of the disease.
Many were the questions that arose during nearly two decades of therapeutic approach with these patients, adolescents, young adults and the elderly, in conducting various research studies on how to coexist with the disease in his existential pathway, what is the influence of their emotions and how family interrelationships, be it positive or negative, interferes with the clinical course of the disease.
This a book aimed at health professionals, for patients with inflammatory bowel disease (IBD) and for those who have some interest in knowing a bit about the psychosocial, emotional and environmental influences in the clinical evolution of the illness.
Este livro é direcionado para os profissionais da saúde, para os portadores de Doenças Inflamatórias Intestinais (DII) e para aqueles que têm algum interesse em conhecer melhor a personalidade desses pacientes e, dessa forma, poder compreender sua forma de ser, seu sofrimento e suas necessidades.
A ousadia de abordar sobre a personalidade e a forma de ser e de adoecer dos pacientes com DII, após atender mais de 5 mil pacientes em duas décadas de investigação e dedicação ao estudo dessas enfermidades, se constitui um atrevimento diante da sua complexidade e etiologia multifatorial.
A grande dimensão dessa investigação nos leva a usar nossa observação e intuição como ferramentas a serem utilizadas. Desvendar o outro, especialmente o paciente com DII, é um desafio uma vez que, como todo o ser humano, na percepção apenas de um recorte, ele nos escapa em sua totalidade.
O objetivo deste livro é o de compartilhar um pouco dos achados que foram encontrados na investigação científica dos estudos realizados com esses pacientes. Considero um privilégio ter tido a oportunidade de conviver de modo pessoal e profundo com o sofrimento desses seres humanos, tão heterogêneos e tão similares em suas características de comportamento e de personalidade.

Escrever sobre emoções, especialmente daquelas de um ser humano que envelhece, não é simples. É uma tarefa complexa, pois o saber tem como premissa básica o fato de não ser estanque e exigir renovação, arrojo e discernimento.
A idealização e a concepção deste livro devem-se à constatação da escassez de estudos na literatura específica sobre os fatores emocionais nas Doenças Inflamatórias Intestinais na área da Gerontologia e Geriatria. Esta lacuna me motivou à pretensão de escrever sobre os distúrbios emocionais, um dos possíveis fatores das DII, que são afecções crônicas distintas, consideradas autoimunes, que atingem o aparelho digestório.

Elas podem ser extremamente graves em sua sintomatologia e necessitam de cuidados frequentes e específicos. Algumas, entre elas as DII, são raras e também designadas como doenças órfãs.

Ao dar início à essa edição, gostaria de prestar minha homenagem de agradecimento ao grande mestre, que sempre me apoiou na importância dos aspectos emocionais junto aos pacientes com DII, Prof. Dr.Juvenal Ricardo Navarro Goes, ( in memóriam) amigo e mentor da minha tese de mestrado, intitulada Retocolite Ulcerativa Inespecífica em Idosos apresentada na Faculdade de Educação da Universidade Estadual de Campinas, Unicamp, transcrevendo o seu discurso como paraninfo da 39a turma de medicina da mesma Universidade.
Esse discurso vem ao encontro do tema abordado nesse livro, uma vez que é pura emoção verbalizada e vivenciada por um ser humano que dedicou sua existência ao exercício da medicina e com quem aprendi não só com o conhecimento médico que me transmitiu, mas também com sua ética e postura pessoal humanista semelhante à minha forma de ser em relação aos nossos pacientes.

To start off, I would like to pay homage to the grand master who always supported me in the importance of emotional aspects in patients with IBD, Prof. Dr. Juvenal Ricardo Navarro Goes( (in memoriam), friend and mentor of my master’s thesis entitled Nonspecific Ulcerative colitis in older people, .presented at the School of Education of the State University of Campinas- Unicamp, by transcribing his speech as keynote speaker of the 39th graduating class of medicine of the same university.
This speech is in line with the topic addressed in this book, as it is pure emotion verbalized and experienced by a human being who dedicated his existence to exercise of medicine. With him , I learned not only with the medical knowledge that he transmitted to me , but also with his ethics and personal humanist stance, which is similar to my way of being in relation to our patients .
When comprehending emotions and managing feelings in a conscious manner, human beings can acquire a powerful tool so that they may find a way to live with a greater freedom in their actions and determinations. This requires the art of understanding the meaning of life and accepting it as it is.
